HOW TO DONATE 1.5% OF YOUR TAX USING THE SERVICES OF COMMERCIAL TAX OFFICE?
When handing over your tax documents to the office for settlement, don`t forget to provide the information that 1.5% of your tax should be donated to the Foundation of the University Children’s Hospital in Krakow „For the Health of the Child” (KRS number: 0000 123 750 and the specific goal: cisco).
SETTLE YOUR TAXES YOURSELF AND DONATE 1.5% OF YOUR TAX!
You can do this in two ways:

Choose a program that will allow you to edit the form field: „Wniosek o przekazanie 1,5% podatku…”.
